Your First Visit

At your first visit, we will discuss your personal history and the problems you may be experiencing. We will do an evaluation to find out where the imbalances and misalignments are in your spine and body. X-rays may be suggested if necessary. You will then receive spinal muscle compression, and a Chiropractic Adjustment, followed by a suggested plan of care to encourage the healing process.

For your convenience, you may download and print the necessary intake forms below. If you prefer to complete these forms in office, please arrive 15 minutes prior to your appointment time.
